When making an appointment, it helps to give our receptionist a brief idea of the reason for the visit in order to assure that appropriate time is given and appropriate documentation can be arranged prior to your consultation.

Once you have completed the enrolment form just drop it off at the practice, please remember to take some photo ID to the practice with you along with the completed enrolment form.
If you have multiple problems and require a longer consultation time please discuss this with reception at the time of booking your appointment.
If you move or decide to change to another GP we can forward your medical files to the new GP, all we need you to do is to give us signed permission to do so. Children over 14 years of age need to sign the permission themselves (this is a requirement of the Privacy Act 1993).

Most practices throughout New Zealand are members of a Primary Health Organisation (PHO). PHOs receive funding from the government to provide primary health services to their communities. By enrolling with our practice you will benefit from these services and reduced charges.
The Ministry of Health website has a guide to the eligibility criteria for funding enrolled patients. Eligibility for non New Zealand residents funding is dependent on your work visa status.
